Nurses and caregivers are essential stakeholders within the continuum of care as the US population ages in the new few years or decades and are well positioned and equipped to expand primary care capacity to implement care coordination, health promotion, disease prevention, and palliative care (Flaherty & Bartels, 2019). There will be a critical demand for a health workforce that can provide quality care for the growing adult population. The use of Collaborative Team-Based Models of Care is emerging and requires the collaboration of healthcare professionals to provide coordinated, patient-centered, and community-based primary and specialty care services in today’s rapidly changing environment (Flaherty & Bartels, 2019).

It is important to provide the necessary resources to educate, train and support the health workforce in their role in the continuum of care for older adults as well as the other key stakeholders such as patients, families, and caregivers to be active agents of their own care. Older adults complex health and social care needs are more intensive compared to other patient populations (Gray et al., 2018). Providing disease management coaching and teaching to older adults, families, and caregivers, including providing patient/family-centered care addressing the unique psychosocial, cultural context, and social needs of patients within practice areas, covers the unique needs and challenges. Involving caregivers as a member of the healthcare teams can monitor outcomes and ensure adherence to key treatment goals while providing the needed support.

The adoption and implementation of models of care focusing on improving care quality and outcomes are significant to determine the best way to measure the effectiveness by assessing service setting, delivery, target patients, and desired outcomes.
